John Michael Perez, age 48 and a resident of Flower Mound, Texas passed away on September 3, 2014. John was born May 25, 1966 in Chicago, Illinois to Armando and Susan (Alvarez) Perez. He is survived by his wife Laurie and 2 daughters, Megan and Emmy. He is also survived by his parents and a brother, Jeff, as well as his grandmother, Grace Alvarez.
